External lab orders (orders from non-UI Health Care providers) can be fulfilled at some UI Health Care phlebotomy locations.

The UI Health Care Phlebotomy Services page details each UI Health Care Phlebotomy location and includes directions, hours of operation, and contact information. Contact your preferred location to verify if they accept non-UI Health Care provider lab orders and if you need an appointment.

Before you visit with an external lab order

For your convenience, please review the below checklist before your visit: 

What to bring:

  • Identification
  • Insurance card
  • A copy of your outside orders

How to prepare:

Patients can bring physical copies of external orders or physicians can fax them ahead of time. Refer to the UI Health Care Phlebotomy Services page to verify where to send orders.

External orders need:

  • The referring institution’s details, including address, phone and fax numbers for reporting.
  • Ordering physicians first and last name, and phone number. 
  • ICD-10 Diagnosis Codes for billing.
  • Full test name, and if possible, the corresponding LAB code from our test directory:

Accessing results

Contact your provider if you have questions about your results.
Test results will be posted to your MyChart account and sent to your ordering provider as soon as they are available. This means you may receive your test results before your provider has had a chance to review them.
